I purchased a 4TB Toshiba drive for my Bolt OTA/Cable. Toshiba MQ04ABB400R 4TB was the recommended drive. I put the drive in my Bolt, let it build and confirmed it was working when completed. Took it out and attempted to use MFSR formatter so I could "TiVo" format it and use all of the drive but I get this error. My assumption is once I run through this I will have the correct format and will be able to use all of the space on the drive after putting it back in and rebuilding it again. I'm confused by the media regions message as it will not reformat the drive.

That's not at all the best way to handle this process, I strongly recommend taking the drive out and starting over.
If you're on TE4, blank the drive, wipe everything delete every partition, put it in the Bolt and let it reformat it, and walk away, you're done.
If you're on TE3, wipe everything, blank the drive, remove any partitions, put it in the Bolt, let it format it, on first boot, remove it and run mfsadd, then put it back in the Tivo and enjoy, you're done.

Yeah, that’s a common snag with the Bolt and 4TB drives. The “media regions” error usually means the MFSR tool doesn’t like how the Bolt initialized the drive the first time. Most people skip letting the Bolt build it first—just run MFSR on a fresh, blank drive, then drop it into the TiVo so it uses the full space without conflicts.

MFSR will NOT work on a "fresh, blank drive". It checks for the unique signature of a drive that has already been formatted by a TiVo before continuing.

I don't know why the OP got that error on a 4 TB drive. MFSR was designed for drives from 3-8 TB, so my best guess is that @zukdj hooked it up with an old drive dock that only supports drives up to 2 TB.
