If you look in Find Programs ... To Do List ... View Recording History, do you see the programs you failed to record there listed as Not Recorded? Select one and does it say "....not recorded because the video signal was unavailable" ?
Do you occasionally fail to tune a channel using the remote, and then are able to tune it by re-trying (tune up/tune down) once or twice?
These are the symptoms of a well known (including to TiVo) tuning failure mode for SDV channels (which are what the tuning adapter is for). I lose up to three recordings a week due to this. TiVo could fix it by just using tuning retries when it detects failure to tune at the start of a recording.
A channel is always retuned (but just once) at the start of a recording, even if one tuner was already tuned to that channel. I suspect when you modify your season passes that triggers another recording attempt (which means another attempt to tune).
You can complain to TiVo but they apparently consider it a Tuning Adapter problem. You might get your cable co. to replace the TA in case that might help.
