Major Sound issue

So I have a series 4 Tivo with a mediacom CC. I don't know if it did an update or a half update but my tivo menus are slightly different. One major thing I noticed is my sound is all jacked up.

I have an HDMI going straight to my TV and I would get audio and video through the TV fine. When I wanted surround sound I would use the optical out to my receiver. Everything worked great for a year.

This week I noticed if I tried to watch a recorded show or a live tv show I get no audio or it makes a bunch of funny noise through the TV. In contrast it works fine going to my AV receiver. And here is the part that is really strange. When I'm in the Tivo menus the audio through the TV just fine for the "Bing".

So what is up with it. Any ideas??
