do I need a tuning adapter for a Bolt and a Smart Samsung TV? The Bolt has a cable card (M crd)

Maybe. But the TV is irrelevant to the need for a TA.

Who is your provider? And are you receiving all the channels in your TV package without a TA being present?

Really depends on your cable provider, some require a tuning adapter (that they provide). It's a cable thing not a smart TV thing.
