I have been using a Tivo Premiere with Cablevision in Morris County NJ since January. I have had almost no issues with my cable card.
I did have reboots from the Tuning adapter, about a two or three a week. I found the simpler solution was to just remove the tuning adapter from my setup. It was only necessary for the foreign channels (ie Indian, Chinese and etc) and some of the HD extended movie channels like HBO Comedy.
So other then the tuning adapter my experience has been as close to worry free as one could hope. And I do hear some rumors at DSL reports that some if not all the Cablevision markets will be dumping tuning adapters and switched digital video after they get rid of analog channels.
