For OTA, start with checking signal strength and move your antenna around.

Check all signal strengths. Do the locals show in the guide, just missing info? You may have to call DirecTV and have them reauthorize the access card.

You can actually do that on D*'s website now. No waiting on hold.

Well I found the problem...

I have TWO of every local channel via OTA in the channel line-up.

one with the call letters all together and one where the the call letters are hyphanated or have a name.

Like:

4.1 WFOR
4.1 WFOR-DT

6.1 WNBC
6.1 WNBC-DT
6.2 Wxplus
6.2 WPLUS

39.2 WDZL-2
39.2 THETUBE

It's like this for 20+ OTA channels. The first one on every option is now checked only and the guide has all the shows and info listed. The other channle just say "REGULAR SCHEDULE."

Should I re-scan the OTA channels? I think this is what caused it last time. The HR10 re-booted and I lost the OTA locals so I had to rescan.

Is this different from what you said in the original post?

No. The program guide was not showing anything (Regular Schedule.)

On the actual channel, same thing. No show info and missed recordings.

Signal strength 88-97 on all of them.

I re-hit the access card and re-booted as well. I will check later if that helped.
I just when to the channel option section and added/deleted the channels.

I'd try a rescan. It shouldn't be showing duplicate channels.

That's always good advice (well, the checking part at least...maybe not so much the moving around stuff if your OTA signal strengh is already OK), but in any case it will have little bearing on an EPG population issue. Since the HR10 does not use PSIP and the EPG is populated only through the sats via the Tribune service, this means that you could disconnect your OTA antenna completely and you would still get fresh EPG data regarding OTA (assuming that process is working properly).
