My DSR704 updated this morning after forcing a phone call. I've been randomly forcing calls for a month and this morning it worked.
I believe the software code was downloaded from the satellite since it only took a couple of minutes. The actual install, from what I could gather, took about 15 minutes.
This Series 2 box was out of service for more than a year and I just installed it last month. It was running 3.1.1 but would not update. I tried buying a movie, forcing calls, doing complete-ground-up-restarts and this morning a regular forced "Daily Call" worked.
I infer from this that the software update bit stream is not always in the satellite broadcast after an initial period of time calculated to be long enough to get all installed units up to rev. This has been suggested earlier by others here in the forums and this morning's experience supports that conclusion.
