If you get an M card you can use one card.

If you get an M card you can use one card.
You can also use one single-stream card, however, your TiVo will revert to single tuner mode (i.e. record/watch only one channel at a time, even OTA and analog)

Why? I should be able to record ota and 1 cable card at the same time. Is there anyway to fix this?

You should, but you can't. The fix? Add a second cablecard.

Without any CableCards, you can record two different OTA channels or analog cable channels at once. As soon as you install one older 'S' CableCard, the TivoHD switches to single-tuner mode, which means you can only record one channel at a time (total).
If you get one of the newer MCARDS (which does the work of two older 'S' CableCards), then that is all you need to support full functionality. If your cable company doesn't yet have MCARDs, then you need to get two of the older 'S' CableCards if you want the TivoHD to operate in dual-tuner mode while supporting digital cable.
