Generally, "hacked" means modifying the unit beyond its original configuration.

For DirecTV TiVos, it means basically enabling networking, and adding TiVoWebPlus, Caller ID, MRV/HMO, and "other" things. At the lower level, it means replacing the kernel with a patched one to disable checks that ensure key system files are unmodfied, so you can modify those files to add said features.

You will read a lot about features that can be added to a Tivo "once it has been hacked." Hack means that initial check bypass.
