How can I ensure that my tivo recordsa show in HD when it is offered in regular and HD broadcasts. Specifically when recording something from a wishlist, should I use HD in the keyword?

The best way to ensure HD recordings is to remove the (duplicated) SD channels from your channel list...

This, however, also has its disadvantages. If, for example, a non-HD show is broadcast on the HD channels - and this happens frequently - then you'll waste the disk space recording the "HD" variant. I believe you'll be better off leaving both channels in the list but check when shows are to be recorded as to which you want. For Wishlists, I have very few that auto-record. For the rest, when a match is found, I check the upcoming schedules to see if it's also on a high-def station and in HD. If so, then I'll manually pick that one to record.
I really wish we could have a multiple-category wishlist - i.e. actor plus a keyword search since HDTV appears in the program description.

AFAIK, the only prime time programs not in HD are reality/competition shows and game shows. If these types of shows are a small subset of your total recordings (and/or you have expanded the hard drive capability of your S3), then I believe that eliminating the SD channels is probably the best way to handle the issue.
If you record many SD programs from HD channels, then you might choose another approach.

For example, I've chosen to auto-record "Lakers". Last night, they played on espn, the SD channel, as well as espnHD. I figure deletely the channel from my channel list would be the best way to ensure that a sporting event is recorded in HD.

This is an excellent work-around, I have been noticing this issue as well (I am still a newb).
I hope this will be addressed with an update somehow.
Maybe it could be able to know that the two shows on different channels are the same episode, but one is in HD. Then we could decide to record the SD or HD version, or even have a way to set it to always record the HD version.

Exactly - they could have a little setting for preferring SD or HD, and they could add "HD only" and "SD only" options.

This is not correct. The amount of space used on the hard drive is directly in relation to the amount of data in content. SD programs have less data than HD programs even if the SD program is on the HD channel. It is the data/show that makes it HD, not what channel it happens to be on. Yes only certain channels can broadcast HD, but HD channels can broadcast SD content at the lower bandwidth(and they do), and that content does not take up any more hard drive space than regular SD content.
