Is DTV supposed to provide a multiswich when they come
and install the 5th DirecTiVo? (or does it depend on which
contractor/installer shows up at my house?)

They usually do supply a multiswitch.

Call DTV and have the CSR add it to the work order. If its not on there when the tech arrives, you may have to wait a while to get it resolved. If its on the work order, then you shouldnt have any problems.

They're supposed to provide anything/everything to get everything in your system running (Including running 2 lines to each DVR).
