There are any number of reasons why this could occur. Try the following to see if it helps:
1. Use a different backup image
2. Try a different CD-ROM drive
3. Try a different drive configuration in your PC
4. Try a different IDE cable
5. Try a different PC
Certain hardware configurations can cause erratic behaviour with MFSTools so experiment until you find something that works.
I use one of my old PCs for working with Tivo drives exclusively. You really don't need the latest and greatest hardware to perform drive upgrades on a Tivo. In fact, newer and faster PCs may be more susceptible to the types of problems you're seeing. This is a case where slower is sometimes better. An old barebones Pentium II or III PC is great for this type of work and can be picked up on ebay for about $25 plus shipping. All you need is a motherboard, small FAT32 hard drive (1-5GB is fine), CPU, 4-8GB RAM, CD-ROM drive, video card, and floppy drive (optional). Just make sure it has two IDE channels.
