intermittent pause then speed up

I have a directv series 1 hughes gxcebot. Several months ago my hard drive failed and I replaced the hard drive using instant cake from ptv. The hard drive worked fine although some of the tivo menu's seemed to come up a little slower. Just recently it has started pausing for about a second, then the video and audio speeds up for about second while I'm watching recorded programs. It is happening at least every 5 minutes. Is it another hard drive problem?

Yes, sounds like classic hard drive symptoms. But you should also check that the Tivo is not overheating .... check its internal temperature under "Messages & Setup" -> "System Information".
