when the Ratio button on the remote will expand the picture to fill the screen?

I have it set correectly in the setup to do so but it does not always fill the screen to (16:9)

It doesn't fill the screen because the channel is broadcasting a 4:3 show centered in a 16:9 screen. In this case you could use the TV control to fill the screen.
