Recording but no sound

I've had a few incidents where a program will record but the sound from part of the program will not. One example was the second Scrubs tonight on NBC: on one Tivo it recorded fine, on the one in the other room the sound from the last 15 minutes was gone. I've had it happen on the Tivo that worked tonight too (i.e. its not isolated to one box.)

Is this common? Any preventative measures?

It's not a common problem.

* brand/model DVR?
* how is the DVR connected to the rest of your system (inputs/outputs, cables, connectors)?
