I didn't find this asked so...
I'm planning on having lifetime service transfered when my S3 arrives but with all the horror stories with CableCard installs I don't want to do that until I'm happy that the cable is working. So, is TiVo Service required before CableCard install or can I wait?

You can wait. I waited, it works. BUT, the new TiVo's come with less than 7 days of free service, so don't wait too long.

I do not know the answer to your question, but I will point out that should you return your tivo after the lifetime transfer, they will "move" the lifetime back to your old unit.

This is the reason that the transfer does not take place until 60 days after you request it. For the first 60 days, the old tivo continues to show lifetime, and the new unit shows 1 year service.
