Separate names with a comma.

mm2margaret is clearly a relatively new and enthusiastic forum member who has acknowledged her errors and is eager to comply with the community ... and that's all great! We learn from our errors.

I think it's better to separate the numbers of the episode from the numbers of the date. I also like to see the episode titles in quotes because that helps them stand out from the show name.
My favorite site for episode titles and air dates is TV.com.
This is now my preferred format. It reads naturally and doesn't lead to confusion (except that some people will tend to switch month & date & year and put in just numbers instead of the name of the month).
I vote against European date formats!
In general when I'm behind on a show the episode name or number are most helpful. When I'm caught up, the date is most helpful. That's a good argument to always include both. I've been too lazy in the past to look up episode numbers, I'll be sure to do that moving forward.
This weeks MM had a very unique issue with the title, but it's not anyone's fault, it gave us a chance to open up the discussion more.
And the placeholder issue has a history, between that and a few other users that do bogus placeholders to make sure all season threads don't happen it can be a hot button issue at times.
Yes, but when I come in here excited about discussing a show, I don't research it first. I look for the date I recorded it so I know it is the correct episode.
